You definitely have to do them separate, and try to do one lower than the other. If you don't you risk cutting off the blood supply to the scrotum, and then you'd have a big problem. Fiasco Farm has great info. about exactly how to do it with lots of pictures. SO helpful! :)
And do you have to crush one side at a time or can you do both at once?
I think you'd be fine without the CDT. The burdizzo doesn't break the skin at all. I thought it was MUCH easier than disbudding, and absolutely no blood at all. :)
